A clear client journey built around secure Google Forms, combined agreements, coach review and scheduling. A 25% deposit is required to schedule your service. The remaining balance is due at the time of your service. No deposit is required for Pathways to Wellness.
Getting started
Enrollment begins with your service choice
Public enrollment is handled through SRI's Google Workspace forms. No website account is required.
Complete secure forms
Intake and one combined agreement form per service are completed directly through SRI's secure Google Forms.
Confirm next steps
Your coach reviews your forms, confirms eligibility and service details, then coordinates payment and scheduling.

Your nurse coach reviews your intake and agreements and confirms your plan.
Pay Deposit
